As I understand, you have to provide PCC of all countries you lived more than 6 months (I think). And it depends on the time you had your name change. for EX: you changed your name in your home country before you lived in any countries then you only have to provide PCC under the uncorrected name...

I dont think so. They used that information for their reports. I think you don't have worry.
