Taking in consideration that everything is done under the EE system. Start EE profile and the system will automatically give you what categories you can apply under and what points will be awarded based on the info you provide.

It's completely up to you. I waited for medical request before sending them. Other have done upfront. Seems to save 1-2 weeks of processing time

That's good enough. if we were required to submit any document for it they would have asked or provided it along with the e-mail btw, I just received an e-mail telling me they received my medical results.

I was required to provide similar info. What I did is I created a pdf file with the information. Provided the following Name, DOB, UCI, Application number, followed by a well presented table of the periods the asked. I haven't heard back from them since I sent it. So I'm guessing it was ok

Hi, I'm in the same situation as you. I called CIC the results at valid for one year and you can use them. I called my doctor to resubmit them for the CEC application and waiting to hear back fom her on Monday

Nope. He wouldn't even tell me if there's any notes written. All he said, "He can ONLY see what I see on ECAS status".. which I believe is not very accurate since some members got useful info out the CIC reps
