I think you should be fine. As the COPR is issued by the government and the Canadian Government does accept it as a proof of PR. Does the documents required for the US visa specifically say that you need to show your PR card??

Depends on your medical results ... If everything is good then you get your Passport request or else you get a letter explaining the reason why you did get approved

It normally takes 1 - 2 months before you hear anything back from CIC after you have done you medicals, again depends on case to case basis. I got received a response from them 2 months after I did my medicals. hold on tight ... :D

That's not strange .... that right !!!! You get your medical request and PR fees together. It does not mean that they r not going to check your medical results. If they find something wrong with your results, they will refund your PR fees. The PR fees are refundable.
