Yes you can use that as a template, but really you just need a letter written from the sponsor. So have a letter written out stating the purpose of the trip, length of your stay and how the sponsor will be supporting you, have it notarized and provide T4's or source of income.

Hey, Thanks for the help, well Im not receiving any help or assitance from the Government or have any pendings against me. So, Im ok for that aspect but do I need to provide my bank statement to show them how I will be supporting him when he firsts get here?
