In consent form, There is Name (printed) written. Anyone has any idea what does printed mean like we have to type and print or what ?

Got it Thanks for the reply. This post was informative. Actually coop isn’t mentioned on the offer letter. Coop is optional, though it is mentioned on my program’s website. So I have taken the content from my programs website itself to provide evidence of coop and attached it with my LOA.
