If you decide to upload the Schedule 4 upfront, you can attach it with a letter of Explanation in that section. If not, they will send you a request while your application is in process. A separate section will be added when this request is sent.

Not necessary but you need a letter from your bank that outlines your average balance for the past 6 months. A lot of people are unable to get such a letter and therefore attach 6 months of their account statements.
