They only require your current passport. No harm in uploading the relevant pages from your previous passport. I wouldn't worry much about stamps in the old passport.

No specific format really. Topic 1 "Regarding {topic 1}, ..." Topic 2 "Regarding {topic 2}, ..." etc. It it's a long letter, you could add an index with page numbers.

I wouldn't be worried about it.. It's not preventing you from doing work, it does't require expensive medication (if any) and it's not contagious.

Looks good to me. :) Most banks don't issue letters as per IRCC's requirements, so your letter(s) do pretty much the same thing.
