Either works. If you have the digital copy, it's always gonna be better than a scan of a printout. No need for the name and other details. Your application is handled electronically, they have your name and all the other details they want.

You are definitely asked to fill in a UCI when you're creating a new profile. The UCI remains editable after you get an ITA. I would say yes, as you will eventually have to fill it.

When you created the second profile, you were asked to enter the UCI, did you enter it? If not, you can still enter it after getting an ITA.

UCI is a unique client identifier which never changes, even after you get your PR and citizenship. So I would say yes, you should use it.
