marlena, I did for the CAIPS. I did not do it for the immigration, because they will not give information on the phone in Egypt - AND I can email as if I am him to ask questions and I have access to his email - so why bother.

You can request the CAIPS notes, but you still need to get his signature on the form to allow release of the information. I don't have the form handy (nor the link), but you can search the previous posts, by doing a search on CAIPS, and you will find a link for the release of auth form, as well...
