Dont be confused about philippine law and canada law. Canada doesnt care about philippine law you are applying sponsorship to canada so they will use their own law and system.

Your name is already in their file or syatem so its impossible to send another sponsorship. if you dont have a plan getting your son in the philippines just make a letter saying that, you dont have any plan getting your son and all you want is to get him examined for your PR application...

I dont think it will affect your application besides VO is looking for a real and fake marriages not the passport last name as long as the person written in your passport is you there's nothin to worry about.

Are you applying for a replacement of your card? This is what I did 2yrs ago, I send the application together with my airline ticket beacause I am going out of the country that time and they issued me the PR card few days before my flight.

Manila visa office is one of the fastest visa office usually 4-8months based on complete application and if they didnt ask you more documents or proof of relationship.

I hope VO accept the new form and include it to my file.
