If you already got MR and PR fee request, the chance of refusal is very thin. The decision made on E CAS could be either before or after sending the PPR. I got PPR yesterday, the E CAS changed to decision made just a few hours ago today. So don't over concern about it.

I agree to update CIC. In my case, I emailed them the same day when the doctor confirmed the upload of the results attaching the info sheets given by doctor and asked CIC to trace the results. They did not replied, but next day, the results was shown in E CAS. Seems it did speed up a bit for me...

You should verify your duties with your employer, if Level A, you need to retake, if Level B, you are good to go. Title is not important from CEC perspective.
