Webform, yes, print it, sign it, scan it and upload it using webform. IF you use account upload, you should verify it and save it then upload to your account. you dont need to print and sign it using this option.

I got the AOR1 on Sept 25 but no SA as of this moment.

I just did mine in Hong Kong, the clinic told me they will mail out the report in 3weeks and should get to IRCC in 5weeks time. Not sure why cant they just upload to the system with the UCI and other info on hand.

dont think you can do it ahead as the clinic needs the IME # for the system

the form says whichever is most recent. 18years of age or 10years... so in your case should be last 10years.

Is the medical report send directly from clinic to Mississauga?

Call them and they said I need to wait and this is a technical problem.....
