No you do not need medical exam for SP extension unless you never done it since you came to Canada (It's not possible because they would never issue you SP without medical exam). Or you might be requested to take it again if you have marked that you have left Canada and live in others country...

Can you apply for Study Permit again? I mean in the worst case, that would help you to buy more time and more chance to get a decent job later. Try to get in PNP before graduating because CIC wont issue PGWP the second time.

It takes around 2-3 months to get it done recently due to so many application was stuck last year.
