For Surrender, go with same name as written in the Indian PP as I did the same for Surrender application & the Surrender cert, which you will receive will have full name. And when applying for OCI you can then use new name as per Canadian PP and name change certificate from the province.

Yes you are correct you dont need to worry about updating Citizenship certificate with new name, as I got the Canadian PP with the new name with old Citizenship Cert & previous name on it. I would recommend you apply for Canadian PP right away without further delay.

Affidavit needs to be from a Lawyer who will write a statement after verifying your name change documents.

Don't upload the one which is not in an official language of Canada, as again they will request you to translate it to English.
