Previous Passport Number section (Line 30) is only when you have expired or canceled Canadian Passport and this section is not for Canceled Indian Passport. So as far as you dont have previously canceled Canadian Passport then you will leave this section as BLANK.

I dont think that there should be any issues with the name of the city. As far you have complete address listed on your old Indian passport then you should be good with it.

Yes you are technically correct but just wanted to prepare my family when they visit India if in case they had a question as why Canadian PP info was not updated on OCI. I reached out to CGI Toronto as well and they confirmed that I can continue OCI on a US Passport or I can also get a new OCI...

Have you updated OCI of your minors with dual Citizenship? Was it just filling Misc form online on OCI website or you had to send in filled application form to BLS?

Yes I went with the same process which you described above. Went with Surrender Cert with Old name and in parallel applied for name change cert with the province. Then updated Provincial IDs & Canadian Passport & applied for OCI with name change cert.
