How long does it usually take between receiving & paying for Biometrics and receiving the instruction letter that allows me to make an appointment to get them taken? I paid my biometrics fee almost a month ago but still waiting.

I've gotten 2 work permits... one approved through the Consulate, one applied for at POE, both times I was asked to hand over proof of education/work experience. Bring it just in case, it can't hurt. if they don't ask for it, oh well... better to have it on you just in case :)

Absolutely do report it. Not only is it against the law, but processing these fraudulent applications also lengthens the processing time for every other legit application, as time spent processing the fraudulent one could be spent processing your own, legit application.
