Not sure what all forms are being shared in this automated internet world. So its better use the same city for consistency till you are inside the province. You can move the next day anyway. It wont be a problem as long as its in the same province.

Either way is fine. They wanted only from 18 if anyone is (for example) 25 but they need only last 10 years. it does not hurt if you give from 18 which goes beyond 10 years.
