No. You can only fill the representative form if you're hiring someone to apply for the study permit on your behalf. Therefore, if you're the principal application and applying for the study permit yourself you don't need to fill or submit any representative form.

When my application got to 2 months without getting any update from IRCC, I raised a webform, that's on the 6th of October, 2023. But there was no reply. Fortunately I received medical request form on 6th Nov. that's 4 days ago.

The refusal would come with a refusal letter to explain why you were denied study permit. Get this letter from your brother or the agent, address those issues and reapply. I don't think paying fees from multiple bank accounts is the problem. I stand to be corrected, though.
