Hi, I received an email asking for supporting documents but I'm not sure if it's a document that's required from me or my partner. In the email it states:

"Dear (my name),

This is in reference to your application for permanent residence in Canada. In order for us to continue the review of your application, we require the following documents:
 (my partner's UCI) - (my partner's name):
Schedule A: Please fill out a separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. The form can be found at the following link (etc etc)
"

Since they refer to my partner's UCI and name on there, my understanding is that he has to fill out and submit Schedule A. However, in the Upload Documents section of my profile, the link to upload this document is under my list of supporting documents, not his. Hence the confusion.

Would appreciate any insight on this. :)
I think probably it’s your partners because they mentioned the his name and UCI, just to confirm call IRCC, btw until when do you have to submit this form ? And did the medical pass today as well, or it happened somewhere in May ?

You're right, I think it should be from him. Thanks! I have 7 days to submit the Schedule A. No update on the medicals yet

I think it's just a standard reply they send to everyone. Although I did get a couple of emails right after that, requesting for supporting documents so it's good that there's something happening with my application.
No additional docs requirement received as of now! Hope things are going well!!

Is there a way to access the web form with the personal information profile (where we input the details of our education, work history, contact details, etc) that we filled up before we received the documents checklist for the PR application? I can't find it anywhere on my application profile.

CIC is asking me to fill out IMM 5669e (background declaration) so I assume I need the same information (dates, titles, etc) that I input in that online profile. Problem is, I can't remember the exact information that I put on there. Is there no way to access or review that information?

Please let me know as if how to fill that form, as I think I also might get an request for it, mainly for question 2 and question 8, will you write your spouses name in native language or how do you plan to add more boxes in question 8 of personal history as there are really few boxes available.

Question 2 is date of birth and question 8 is about government positions, right? The name should be however it's written on the passport copy you submitted with your application. If it's written in the native language on the passport, that's how it should be written on the form as well. And yes, you can add more boxes - there's an option to "Add more rows" below each entry

Really did you download eimm5669e or IMM5669e ?
I checked both the forms one lets you enter the boxes other one doesn’t ..
the IMM5669e’s Q8 is personal history ..
Make sure that you upload the form which they specifically asked for ... both are same forms but a little bit of a difference.

EIMM5669e. It's the form they specified in their request letter :)

Oh then it’s great .. it’s easy to fill as compared to the other one IMM5669e...
btw how do you plan to sign it by handwritten signature ? Filling , printing and then signing it ?
