I am filling NEW ATIP form (IMM 5744) and feel super confused. I filled and sent IRCC the form twice and they still asked me to refill and resend.

I am Hai - the main applicant and my spouse is Minh - the dependent. I am requesting IRCC to provide GCMS for my CEC - Express Entry application. My questions are:
1- Section 1. Designated Representative's Information => This section is for me - the main applicant to fill, CORRECT?
2- Section 2. Applicant's Information => As long as the guide, this section is for my spouse, CORRECT? Do I need to fill and sign in this section at all?
I notice there are two columns. The first column on the left => Is this for me - the main applicant to fill and sign? (there is no "Relationship to applicant" space here as in the right column).
The second column on the right => Is this for my spouse to fill and sign? Or the first column on the left is for my spouse?
I searched in vein for this forum but the discuss is about the old IMM 5744 so I am totally lost now!! Appreciate any expert help!!!!!

I don't know why he should be pissed. Don't take the matter to heart, some call agents are typical civil servants.Called CIC agent now I am more worried than before. He appeared to be pissed that I called and said that this is the third time I am calling (September last week, November middle, and today in December) and everytime a call is made they mark/flag it.
He also asked me if I was being told when to call next time when I called CIC last time. Also pointed out that I have requested a gcms note last Friday.
Reluctantly checked my file and said Eligibility is completed (i know from november that it is passed), Criminality passed. It is now processing for security. He said my case is complicated and can take anywhere between now to April 16th 2019.
He also asked me whether I got arrested in Canada or in my country of citizenship. I never did. I don't know why would he ask that. Looks like they are security screening me or something. My record is super clean but this makes me nervous.
Bottomline is, he is super pissed at me because I have called 3 times since I made the application and ordered 2 gcms. I behaved very politely though.
And, looks like I have to wait until April 16th 2019.

I applied for my second set of notes as well... even though I haven't received my first set yet ( I applied for that one sometime in September)And just like that another day has gone by! If we didn't hear anything by now it is highly unlikely we will have any movement in rest of the day.
So, I'm now waiting for Tuesday. And I applied for second set of notes just to get the feeling of "doing something". Two more days to go!!

Btw, You're the fourth Person on this forum who has been stylishly given a 10 months timeline (4 months addition to the 6 months) and I don't think it has anything to do with security screening. I think CIC is considering extending the timeline and a few agents have been telling applicants (whether it's intentional or not, I can't say).
Also, there are people who are still months away from the current 6months timeline that call CIC biweekly without any implications so don't fret about the implications of calling. There aren't any.
Just keep your fingers crossed like we all are and I hope that you'll get some good news soon.
The only hope that I have for bulk release is that if IRCC employees are super happy during the Christmas - New Year phase and decide to skip a few process to directly roll out big batches of PPR/COPR's.Completed 180 calendar days today!
Hoping for a miracle in next week for all of us. Someone had pointed out in the past that even in 2017 there were PPRs issued in bulk in last 2 weeks of December. Hopefully, the agents will be impacted by the Christmas cheer and they will decide to make this Christmas and new year special for us.

I repeat, except 24th, 25th and 26th Dec last year, there were PPRs issues on all days of December.
