Additional dependent information have to provide in Generic form by clicking to spread. No need to fill separate form, it is needful for very huge families.
Additional family information form is only applicable for major family members. Not for minors.

Yes. And She is the main applicant particularly in that form only. I think, have to use her father"s surname with her name, not your surname (Maiden name) Each name you have to write in your native language by pen in thin letter in front of the printed English written name, in the same line.

i have a question regarding the translation affidavit, who should sign as a public notary? is it OK for a lawyer for example?
