Citizenship online application questions

Hi, I'm preparing for applying online for the citizenship and have a few quick questions, I have not created an online account yet since it's timed, but would like to know the following in advance to better prepare the application. Many thanks in advance!

1. If the applicant is more than 65 yo at the time of application, do they still need to submit CIT 0116 the waiver of language/knowledge test, or is it automatically waived without that waiver form?
2. For the paper application checklist, the applicant is required to submit color photocopy of all passport pages during the 5 year eligibility period, does that also apply to online application also? If so do we color scan all pages to submit online instead of using physical color photocopies?
3. Do I still need to get a police certificate (from Canada?) if I have been physically residing in Canada 100% of the time during the 5 year eligibility period?

A follow up question:
It seems the paper application form (cit0002e-2) is asking for the income taxes in the last 5 years, is that a recent change on the requirement? I thought on CIC's website it only mentioned of the most recent 3 years of tax docs out of 5.

It's always been 5.
