Thanks a lot for sharing! Can you or someone here can also please clarify if this shows mistakes made or should this be considered as corrected information in ‘Red’ font? Sorry, I got a bit confused if the applicant passport details should be as per the Canadian passport as mentioned in this link OR should it be per Indian passport since this link shows general mistakes made?

That's strange. If you check link as of today, they (should) charge $60 for out-of-province application whereas for within Province its $40, not sure if it includes anything "value-added" services or stuff: vas_canada.pdf (blsindia-canada.com)The cost of the postal application service is now $62 each way.

Sorry, I am talking about the applicant passport info section (passport no, issue date, expiry date, place of issue) - if I should fill my canadian passport details or indian passport details?
Since I found 2 different guidelines. BLS step wise process (https://www.hciottawa.gov.in/pdf/Step-wise-process-for-OCI-application-new.pdf) mentions that it should be indian passport details. Whereas, the link you shared (https://www.blsindia-canada.com/toronto-jurisdiction/pdf/OCI-Sample-form-with-common-errors.pdf) says it should be canadian passport details.

if you dont wanna waste money at BLS who will end up charging you $60 to redo your application form.

Is it ok to send OCI from Canada to Indian? Do FRRO care about how you got the OCI card from Canda? What Is OCI registration certificate? Is this change of status from visa to OCI without leaving country?

Don't courier/registered post it would be my suggestion.
Even if you wanna send it send it through someone whom you trust and is traveling to India
If the intent of your friend/family is to stay for a longer duration that of eVisa/Visa can provide then sending the OCI makes sense. Otherwise it does not.
