I think you may have been asked by IRCC to the correct the same form that I have been just asking to fix and resubmit. Mainly just trying to find out which IMM5669E form is right to submit via email as your form submission seems to have worked out.

My mother completed the medical here in Canada and the clinic gave a two page - emedical information sheet and asked to upload it to my mother application. We applied online and i am not sure how to upload this document.

Can anyone assist me with this please? Where and how do we upload this document. This is not the test result, it just says that my mother had attended the medical examination in a emedical clinic.

There is no need to upload proof that she attended the medical. Only thing IRCC wants comes directly from the clinic. Just clarifying that the appointment was done in person and that is just the name of the clinic.

Looks like for whatever reason, my application is an odd one out.

Got biometrics request around 6 April but no medical request or sponsorship approval as of now.

For everyone else, the 3 go hand in hand.

It is not sponsorship approval it is sponsor approval. Could be for many reasons. The process is 2.5+ years so as long as you get it eventually it is fine. Many are still waiting for AOR.

You will soon receive and update. That happened to ours f12 updated was 4/8/22 then after 10days they emailed my husband and requested our option c 2018-2020

I am trying to have my option c printout mailed to me in case IRCC requests it. I am using the phone number
1-800-267-6999 but what I get is that the process can not be requested. Anyone else had the same problem?
What can be done? ( I can print the 2020 and 2019 online but not the 2018)

I requested option C printout from CRA for 2018 via the same phone number. True that 2018 option is not available thru automated system. What I did is just keep choosing option which direct me to speak to any agent. It was long wait time so I chose option to call me back by them. They called me an hour later and I was able to request 2018 option C printout. It arrived in mail last week. Now I do have 2018 printout and will download 2019 and 2020 incase IRCC requests. Hope this helps.

Hi guys,

So finally I heard back something from IRCC almost after six months from application submitted on 27th Oct 2021.
They requested to resubmit the IMM5406 form for principal applicant (my dad) and eligible dependent (my mom). I have a few questions related to that with hope that experts will guide me better

1. Should only my dad and my mom has to resubmit the form via email or being sponsor of them I have to fill the form as well?
2. I have a sibling who is married and not a dependent of my parents any more. Does she has to fill the form as well?
3. I am my parent's authorized representative and I received that email on my email id. So should I have my parents fill the form and signed it by hand and resubmit the form from my email id?
4. PR portal says application is "returned". I am able to see that along with IMM 5406 and consent & declaration also shows "incomplete". But there is no mention about consent & declaration form on email letter I received, so should I worry about it?
5. F12 shows the status 8 - returned. So after resubmit forms via email, how long does it take to change the status?

I know many of you have been in same boat at earlier stage and have great experience in this scenario. So your inputs will help me a lot here. Thank you all _/\_

eIMM 5669 Schedule A: Sponsorship Background/Declaration Form – To be used only when submitting a sponsorship application or when applying for refugee protection within Canada (Guide 5289, 5746 or 5772)

When you download file , the file name look like EIMM5669E.PDF

Hi All,

I received an email letter for my parents sponsorship application (Submitted Nov 22, 2021). I unfortunately left 2 months gap in my father's personal history. I have been asked to redo the form 5669e and submit via email. However this form was originally completed online through the portal, now they've attached a link to a PDF form that they are asking me to complete. .

Few questions:

1) Should i stick with the form they have linked in the letter or use the slightly newer one i found on google? One thing to note they are named slightly differently. One is eIMM 5669e, (11-2018) the other is IMM 5669e (06.2019). Any idea which i should go with?

2) The form they have linked does not have a place to sign at the bottom instead has by typing my name I am electronically signing my application type field. Why are they asking to hand sign there then, the other form i found on google does indeed have sign here spot... This again leads me to question if they sent me the link to the right form...

3) Also does hand sign have to be print, sign and scan or can it be a digital hand sign? i.e. sign through iPAD etc?

Anyone who got a similar request regarding fixes in 5669, which form did you end up submitting?:

IMM 5669 (for Principal Applicant): The Schedule A (IMM 5669) is incomplete. Please ensure that the form has been completed with details of your Personal History Section 6 and/or Address History Section 10 since the age of 18, or the past 10 years, (whichever is more recent) and does not contain any gaps. There is missing personal history and/or address history for the following time period: 2017-08-01 - 2017-11-30. Please provide a newly completed IMM 5669. Please ensure to complete all questions of the form. Please ensure that you sign the form by hand and date it once it has been printed. You must submit the most recent version of this form. https://www.canada.ca/content/dam/ircc/migration/ircc/english/pdf/kits/forms/eimm5669e.pdf

Answer for question 1-eIMM 5669e

2- this form eIMM5669e - send them 2 copies. 1 with digital signature only and 2nd with digital signature then print out pdf form, Hand signed it, then scan it and save .

3. Send a explanation letter with email in word or pdf format for why you send 2 copies.

I did the same , they know this form don't have a space for signature but to be on safer side and Avoid any delays, I send all above , 2 copies of eIMM5669E.pdf and explanation letter.

Note- My case I m sure I did not left any gaps when I filled up online form IMM5669 on PR PORTAL. But My parents was Government employees so may be they wanted to have a form that hand signed as these forms contain that info. So they might requested physical form.
