Work Permit Application

Hello Everybody
I was just wondering what link I need to go to fill the work permit Application to be sent with Inland PR Application??
I will appreciate if somebody can post the link here....

You need to print out the OWP application (form IMM5710) and mail it WITH your Inland application (don't try to submit it online).

Be sure to write across the top of the first page:
OWP SUBMITTED WITH IN-CANADA SPOUSAL SPONSORSHIP APPLICATION. PROCESS AFTER AIP.

Can I send my extension application again when my pgwp is close to expire? Does CIC mind it sending twice???
Just for clarification I have PG Open Work Permit valid upto February 2015
