For Job 1 - I was hired by company A located in M city , but contracted at client company B in Y city.
After one year was again contracted to client C in Z city.
1) In the reference letter from company A ( who hired me and paid me ) , should the client company name and locations be mebtioned with the text saying “ she was contracted at client B and C companies” ?
2) Should two different entries be added in the work history and personal history in this case with locations Y city and Z city or the job location should be city M for both these job episodes ?
Please advice.

This is how I would've dealt with this:
I would've added the client companies in the work history/personal history sections instead of the company who hired you and include the name of the company that hired you under parenthesis. I would have included a reference letter from both the client and the company who hired you as well as a LOE to avoid confusion.
Hello, we claimed points for my spouse’s 1 -year Canadian work experience. He holds OWP since I was a graduate student. To prove his experience, would the letter of experience + pay slips + T4 and notice of assessment be sufficient? What would you advise? Thank youThis is incorrect. OP's spouse is working in Canada so there is no continuity requirement under the CEC program nor is she required to have her one year of work experience under a single NOC.

Obtaining letter from the client at this point is not possible as this is an old job . I only have reference letter from my employer. Shall I ask the employer to add client locations in the letter ?
