I don't think you understand, there is no such thing as a "5 year period". The time to meet RO is the previous 5 years from whatever day it is today. So for example if you applied for a PR TD in Oct 2016, CIC would check your RO going from Oct 2011 - Oct 2016. It's always 5 years from any date. With your family medical condition you could try to apply for PR TD on H&C grounds, and submit all proofs of his condition (doctor notes, hospital stays, etc). It's up to CIC whether to grant you the PR TD on H&C grounds or not. If they reject it, you'll be given chance to appeal. If appeal is also rejected your PR status will be revoked.
If you have a wife in Canada that meets the RO, she can always sponsor you again as spouse under family class if your PR is revoked.
