I got MR Feb 11 and done on Feb 12. Doc uploaded my medicals on Feb 22, still waiting for PPR :(

no need to worry about ECAS, I don't think it's updated regulary. If you see my case, it's still early for you to worry ;) :)

^ I agree...my suspicion is that they update in batches.
If you follow the threads from this forum, some had PPRs even if ECAS was still In Process. Others had PPR a day after the Medicals Received came-up. Still others had Medical Received but waited for X weeks/months before getting the PPR.

applied gcms notes today,(hopefully I will get ppr before gcms notes)

If you read FAQ from e-cas it says it is updated daily but...also says (quote):
"(CAS) may not show some activities on your file for many weeks. That is because the visa offices and the case processing centres get a lot of information.
In many cases, an in-process file is actually waiting in a queue for initial review. Applications are handled in the order they are received and on a case-by-case basis. The waiting time in this queue can be long. Applications are processed according to the application processing times at a given location.
In addition, CAS does not show some other actions—such as security checks—at all. So it may appear as though nothing is happening to your file, even though you have had interactions with Citizenship and Immigration Canada recently."

how can they send a medical request for a file which is in Initial review? I think e-cas changes to "in-progress" after Receiving Medical request.

in my case, they even sent RPRF & Biodata request very next day after medical request and nothing after that from 2 months.
 
After four weeks of impatient wait I finally got my PPR email yesterday at 10:30pm EDT
 
today I noticed, ecas is updated 'Medicals have been Received'

Medicals requested on: 6th March 2014
Medicals done on : 18th March 2014
Medicals Uploaded by doctor : 24th March 2014
Medicals reflected on ecas: 11th April 2014

do we know how long does it take to get ppr, after medicals are received by ecas?
