Write your net personal income for the 12 months preceding the date of your application. Also write the dates of that 12-month period.

it states to give your net personal income. So it's an estimation of how much you've made for the last 12 months. As others have mentioned, unless you're applying from Quebec you will need to attach the Option C anyways and that's what they're ultimately looking for to see if you've been on welfare. Since you are sponsoring a spouse you don't have to have a certain income to sponsor (unlike some other Family classes).
Really read the guides thoroughly as they are mostly self explanatory.
