What if CIO or Visa Office misplaces or loses our documents

Is there any documented cases where CIO or Visa Office misplaced or lost application documents? What will happen if they lose our documents, will there be compensation of any sort. Also what can be done to avoid documents being lost or misplaced once our document reaches the Visa Office.

There's no compensation. If they lose them it is simple, you have to resend lost documents again.

I have heard that UK Visa Department has a heft compensation, if they screw up at their end. :'(

Why would London office had that kinda problem?
