It should not be identical though.

That form I linked is no longer issued by CIC since last year, marked as CIT 0171 (09-2012). But the updated form has slightly changed requirements to period which documents or information should be provided for. That's the most interesting difference and important change between versions.

There should be CIT 0171 (10-2013) at the bottom. If you have it, please upload it, it'll be useful. CIT 0514 is likely just a cover letter for RQ.

The Linked you shared referes to the possibility of requesting information about what could be the specific concerns from the CIC.
Is that possible? If so that will be very useful

I don't think so. You either get RQ or document request form.
