I don't know why but the vast majority of ppl waiting for MR are October applicants

I wrote my previous post before noon during my lunch break at work. A few hours later, I saw the e-mails in my mailbox! They came at around 3 p.m. They weren't there when I was writing the post.

Dear Warsaw applicants,

I've received a request for updated Schedule A for myself and my wife instead of MR.
Since the time I've applied for the program, we have a baby now.
I would like to send the paperwork for my child along with the updated Schedule A.

- What documents do I need to submit for my child?

Thank you for your responses!

First you should mail them informing them of the birth of your child.They will reply you within 3 working days and tell you the documents to send.

These are the documents they will tell you to send:

a certified copy of your child's birth certificate along with its translation into English or French by a sworn translator;



-- 4 recent photos for your new born child ((for Photo Specification please consult http://www.cic.gc.ca/ENGLISH/pdf/kits/guides/3910e.pdf Appendix B))



-- Specify height and eye colour for your new born child

a copy of the child's passport

add your baby to the Application for Permanent Residence in Canada and you and your spouse should update the IMM5406 Additional Family Information form. You will find the application kit at the following website address:http://www.cic.gc.ca/english/information/applications/skilled.asp

-- pay the processing fee for you newborn child in the amount of $150CDN

What does "certified copy" mean? I guess we only have a notarized copy in my country. Can anyone please clarify? Thanks.

Guys who got their MRs, how does your medical form look like?
The form that I got has only two pages, the first page has instructions, the second page has my details and a section for the doctor to fill in with only two boxes "serious disease" and "no serious disease". Is that right or is the form missing pages? I expected something more extensive, such as a medical questionnaire.

BTW, how does the medical exam look like?

Apologies as i was travelling and could not update.

Received PPR on 27th Nov from WVO. Submitted the PP to NDVO on 2nd Dec and got back stamped visa on 4th Dec. Planning to land either in Jan or March next year.

That is the medical form, print and take it alongside to the hospital for you medicals
