I wrote my previous post before noon during my lunch break at work. A few hours later, I saw the e-mails in my mailbox! They came at around 3 p.m. They weren't there when I was writing the post.:D

-- 4 recent photos for your new born child ((for Photo Specification please consult http://www.cic.gc.ca/ENGLISH/pdf/kits/guides/3910e.pdf Appendix B))



-- Specify height and eye colour for your new born child

a copy of the child's passport

add your baby to the Application for Permanent Residence in Canada and you and your spouse should update the IMM5406 Additional Family Information form. You will find the application kit at the following website address:http://www.cic.gc.ca/english/information/applications/skilled.asp

-- pay the processing fee for you newborn child in the amount of $150CDN

Guys who got their MRs, how does your medical form look like?
The form that I got has only two pages, the first page has instructions, the second page has my details and a section for the doctor to fill in with only two boxes "serious disease" and "no serious disease". Is that right or is the form missing pages? I expected something more extensive, such as a medical questionnaire.

BTW, how does the medical exam look like?

It is the same thing as notarized copy.

Apologies as i was travelling and could not update.

Received PPR on 27th Nov from WVO. Submitted the PP to NDVO on 2nd Dec and got back stamped visa on 4th Dec. Planning to land either in Jan or March next year.

I would like to thank forum members and hope that you get MRs/PPRs asap from warsaw visa office. Spl thank to Manchi83

That is the medical form, print and take it alongside to the hospital for you medicals
