I'm Applying visitor visa for my in-laws, Me and My wife are PR holders in Canada.
My wife's due is in Aug, Trying to understand what is "site visit authorization form" and where can we find this. In which situation we need to fill this.

Hello all,
To apply visitor visa for my in-laws, do we need to have separate PHAC letters, or one should be fine.
If we need two, can we submit 2 phac’s with the same UCI number as I’m going to submit single application for both.
Please advice!!

Biometrics requested: December 8th 2020
Biometrics completed: December 17th (Dubai)
Passport Request: Dec 31st 2020

Don't worry. The other will also come. When I applied with my family we got the passport requests with a gap of 2 days

Hi, my PHAC approval got rejected because the place for the quarantine is same as the location where care the patient is required. They've asked us to mentioned that on form and include site authorization form. Where on the form you should mention that both locations will be same??
How can we get a site authorization form and who can approve it ? I too got same reply today from PHAC
Me too was calling my mother for postpartum support and so left the site authorization section blank as we don’t intend to have seperate location for quarantine and site of support. How can we resolve this ?
