Hi! My cousin, who has a visitor visa here in Canada wants to apply to Us as a visitor. Can she apply directly to the US embassy? What are the documents she will need to present? Appreciate any advice. Thanks.

No documents, she has to schedule an interview at the nearest consulate (or embassy, if she is in Ottawa) after submitting the DS-160 online and paying the fees.

Thank you. Do you have an idea how much is the fee? I heard, $160.
 
You can just as well verify on the Department of State's website, but yes.

Just a word of warning that while this is allowed, it's hard to establish her ties to her home country while she is applying for a US visa as a visitor in Canada.
