very urgent regarding FSWP

Hello ,
plz give suggestions:MY VO IS POLAND.i HAVE APPLIED FROM INDIA .
1)They demanded PCC with wifes maiden name.Passport PSK refused to this ?How can i get Pcc with current name and maiden name.
2)How should i send them rprf payment receipt.

dont worry....i have the same issue but u can get PCC from CID local OFFICE by giving out simple application and docyments to them....its shame that passport office is not giving out PCC due to maidan name.....but the best thing is to get it done from a local CID office.....
