Use of Rep form sign by parents

Hi there, I am filing up the application for my parents supervise, I wanna ask do we need to fill Use of Rep form and do my parents needs to sign the form as well, can they sign it digitally or I have to send the form and they have to sign physically and send back, what is the easiest way they can sign the form ?

What are the section. and information I have to fill for use of rep form ?

you have to fill the part that refers to you as representative and send it to your parents, they print , sign and send you scanned copy. that's all .
