Urgent question about "other name"

Hi Guys,

I am finishing up the PGP application and in the IMM1344E form, there is a question about sponsor (me) if I have ever used any other name. Now, in my passport I have only one name (first name) that I have already put in the form but in my employment letters (even in CRA) I have a first & last name. So, should I answer this question as "YES" and enter the other name that I use? Do I need to provide any documentation for the other name right now?

You need to use full name on the forms, i.e first and last. So the answer is yes and you better to provide explanation of the name discrepancies as well.

other name can be your pet name if any or woman before marriage so no specific document required for that
