On my Birth Certificate, I do not have my last name. But all my other documents and passport has last name.
What should I do?? Please help..

I have already got my birth certificate translated into English. I am just wondering, If I submit it like this how it is going to impact or is there any other option that I should use.
 
write an explanation letter and attach it along with your birth certificate or send any of your education document which has your parents name written.

Just a query... why do you need your birth certificate? I haven't used that since I had a passport - the only government issued ID they've asked for. Have they specifically asked for your birth certificate?
 
for OOPNP they require birth certificate so I guess this issue is for that thing. but write an explanation letter with your passport and transcript copy attached.
 
Here is one more interesting thing- My father's name spelling is different on my passport and on my transcripts.
I am so confused.
 
Hi friends does the spouse also need a birth certificate in oopnp or just the principal applicant
 
We need birth certificate only for children, primary and secondary applicants don't need this, it not asked anywhere in the process. Don't worry.
