My cousin has mailed his passport for visa stamping to Islamabad today, and then realized that he forgot to sign the passport! Does anyone know what might happen? Will they return the passport?

Don't worry, I don't think that it would be an issue at all. Signature can be important while you are traveling abroad but it has nothing to do with PR visa. I have seen many people who travel abroad many times without any signatures. So if his passport is authentic then he should not worry about anything.
