URGENT: Form MM5406

My application was just returned because I forgot to sign form 5406. Quick question please.... My partner is my sponsor. He is a Canadian citizen. I am the only one migrating. No one else in my family is migrating and I do not have any children. Do I still need to list out my parents and siblings? I just don't one the package sent back to me. In addition, the MM5406 I filled out looks slightly different than the one I found on the site again. Should I return the one sent back signed where requested or fill out the new version. HELP please!!!

I heard that a few forms have changed recently, but it sounds as though they have just asked you to sign it and send it back (especially since they have specifically indicated where you are to sign). Did they say in the letter that you have used an outdated form?

IMM 5406 is for your family information. CIC want to know who your family members are so you must tell them. The additional family information is not related to who is immigrating with you so you need to list your parents and siblings.

You need to download and complete the latest versions of the forms. Any forms that have been updated will need to be redone with the latest version.

Thanks to both of you for your responses. I am going to complete the new version of the form and add all my family info as requested. My other doubt is whether my partner who is the sponsor also has to complete form 5406. Please advice. Thanks so much!

No, your sponsor only has to provide the information that is in the sponsor package. If you read the guides on the CIC website and follow your country guide you can't go wrong.

IMM 5406 is only in the applicant's pack.

If the whole application was returned, when you resubmit it, it's treated as a new application. You should use the new version of forms if they have been updated.
