Submit your application and wait for your acknowledgement of receipt. After you receive this, go ahead and do upfront meds if you like. Remember if they expire or if you are rejected the money will be wasted.
I am the main applicant and with my dependent child we are in Canada. My spouse is in home country. Does the medical request comes separately for main applicant,dependent and spouse? If we both (main applicant and child ) go for upfront medical will my spouse have any medical request or does he need to go for upfront too?

Do not do upfront medicals.. you may think your application will be processed fast.. but its not true..
There are many applicants facing issue on this forum bcz of upfront medical, CIC does not find reports .. delay in PPR request.. etc.
