I just got an email from doctor that my medical has been sent to CIC and doctor send me attached completion of medical . As per above post I m little confused . When i went to get my medical done doctor gave me a sheet and I have also received one today.
My question is which sheet i will be sending to CIC.

E-medical information sheet contains your UMI number and this is specific to medical; should be sent. Since you have both, it wouldn't hurt to send both. When I did mine, I also mentioned my doctor's name, address and whatnot, although I didn't have the confirmation page you talked about.msingh199 said:Hello there,
