Up front Medical Exam

My wife did her medical in Bangladesh just a few days ago. The doctor gave her a copy of "Up-front Medical Notification", e-medical. But in the document checklist I am required to attach the Imm 1017 (Client bio data...) medical form.

I am confused why the doctor gave her a different form. Any problem to submit it? Anyone had the same situation?

I don't know if it has something to do with the change from paper medical to e-medical. They changed the whole process in March 2013. The paper medical form was called IMM 1017 but maybe the name of the form has changed since then. Did you check the document checklist on the CIC website again? Maybe the name has been changed now and it's not called IMM 1017 anymore

With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:

IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)

i have the same confusion as my children had their medicals at IOM centre in manila and all they got was an eMedical form filled with their infos therein. Do they need to be issued an IMM 1017 form? If so, then why did the clinic did not hand them in to them after they were done their medicals?

I need their proof of medicals ASAP so I could submit my application to sponsor them here in Canada.

Can somebody help me what to do?
