Posting this here in case it becomes helpful.
I tried analyzing my notes and here's the info I got. I'll group the details by the timestamps in the notes section at the last part of the document:
I ordered my first notes on May 16 and got them on June 16.

My AOR is Feb. 12, 2016:

Feb. 19, 2016
-case parked due to ff. missing documents: schedule 4

Feb. 22, 2016
-sent for R10 review
-check of min. score against ITA score
-no representative
-medicals attached
-client fees: paid
-IELTS: met threshold and verified
-other documents set for review notably police certificates and diplomas/degrees

Feb. 23, 2016
-R10 OK
-criminality decision passed
-ECA report verified on issuing agency website; foreign credential confirmed on ECA site
-AGE: no change from ITA to e-APR

April 29, 2016
-A11.2 met - applicant meets threshold
-Educ: met - foreign ECA
-after this other checks like RPRF: paid, civil status, nomination, does not hold valid status in Canada, and PNP nomination
-Work experience: duties listed in the reference appear to be consistent with the actions in the lead statement under NOC code selected; substantial number of the main duties under the NOC code selected; the letter of reference appear to demonstrate that PA met min. requirements of 12 months of work experience
-IELTS: meets threshold

For movement between offices, based on my analysis:

From CIO to CPC-Ottawa: Feb. 24
From CPC-Ottawa to my VO: May 03

I read that you can raise a CSE addressed to NDVO. But thing is in CSE it never asks which visa office this is directed to.

I ordered my 2nd GCMS note on 20th June 2016 through one of my friend in CANADA using ATIP website (https://atip-aiprp.apps.gc.ca/atip/requesterInformation). Yesterday he received an e-mail from IRCC:

This is to acknowledge receipt of your request submitted pursuant to the Access to Information Act received by Immigration, Refugees and Citizenship Canada on June 20, 2016 for the following records:

Dont worry. This happened to me. When ordering gcms, in the section of what notes you want to request- you have to select permanent residency instead of express entry. This mail came because you selected express entry. Just reply to this email saying you would like to abandon this particular request so that they can close that request and now go order a new set.

Instead of Express Entry, you had to choose Permanenet Residency as your application type.

Now, you can just reply to the email and state your intention to amend your request to access Permanent Residency File.
