UCI clarification

Hello there,
I need some help. My mother has a history of Canada application.
In 2000 my mother was primary application and landed with our family for immigration - there was a UCI on it which we cannot find.
She couldnt get PR status as she had to reside outside.
Eventually she lost her status.

Last year she got Supervisa approved and there was a UCI on it.
Anyways now I golden email to sponsor through PGP.

Should I put the Supervisa UCI on it or leave it empty in form IMM 0008 ?

Use the one that you have, not the one that you don't have.

The text above about her 'history of Canada' is completely unclear, but make sure you disclose that history in her PR app; if there's anything you don't have, disclose as much as can, and explain the rest (including what you do not have).

Hello when you say "make sure you disclose that history in her PR app", like disclose where? Can you elaborate where the information can shared?
It is very straightforward. My mother was primary applicant and applied to Canada as a family (gave IELTS and got accepted in draw) and got her PR and landing papers.
She landed in 2000 with her spouse and children.
But in 2000 she could not afford to stay back in Canada and complete the years to get citizenship, hence she lost her PR status and no citizenship application was given.
20 years she worked in her home country often visiting Canada for a month or two.
After 20 years finally all her kids have citizenship so now we want to apply through PGP.
In 2000, everything was paper based, so we have no UCI or application or any documents.


Ive read this guide thrice, there is no place to share details about PR accepted 20 years ago or forms to fill this information. Please correct me if there are specific forms to declare this.

I believe there is a section/question where the applicant is asked about 'immigration status' and citizenships held before. Provide info there.

I would suggest including a short letter of explanation that she landed as an immigrant in [year] and renounced it/had it revoked because could not remain for personal reasons.

I presume not possible to have her [ex?] spouse sponsor her?
