Translation of documents

Hi,
I hv got the translation of my birth certificate from a notary in India. Under Translation column its mention we need
the English translation; and
an affidavit from the person who completed the translation (see below for details); and
a certified copy of the original document.

The Notary guy who did the translation, not able to understand what to write in affidavit.
Please can anyone help me with this?

As far as I know if it’s noterized it doesn’t need an affidavit.

My husbands papers were all translated and certified with copies of originals and everything was fine.
