Translation of Documents

Hello All,

I live in Germany and my Code of Conduct or Police Clearance Certificate is in German. I will get it translated into English from the Certified person. But do I need to also provide an Affidavit from the translator?

If yes then is there any format for it? Please help.

If the translator is a member CTTIC you don't require an affidavit: http://www.cttic.org/certification.asp
