I also had this question in my mind. But my assumption is, medicals reviewing and application processing departments work independently. This is just my assumption as I saw people updates on ECAS, but it may be different in reality.

Yes you can pay your RPRF upfront. I mean you can pay it now. Please go throu the below link and follow the instructions.

I believe, it gives you answers to all your questions while you go through this link. Personally, I did not go thru this link as I have a representative and they have already paid in behalf of me. So, honestly I don't know the whole procedure. But I hope this website gives you all the required info.

As long as you are an in Canada applicant you can send this receipt with your RPRF request to the VO processing your application. If you are an out of Canada applicant you can't use the online receipt process. If you have you need to get a refund.
