Pa advise naman po. medyo confused lang po kasi ako sa FORM IMM5406 E - Additional Family Information. Nakasaad po kasi dun na "The principal Applicant, Spouse and all children 18yo or older must their own copy of this form...)

Since 2 lang po kmi ni hubby ang applicable,panu po ba pag fill up nito?

IMHO - Hindi naman nagpalit kung sino ang Principal Applicant at sino ang declared Spouse sa application di ba? Sino ang nagapply sa nomination at sino ang kailangan mag submit ng application package sa CIC?

Who needs to fill out this application form?

This form must be completed by:

You, as the principal applicant,
Your spouse or common-law partner (whether accompanying you to Canada or not), and
Your dependent children aged 18 or over (whether accompanying you to Canada or not).

So hindi nagbabago kung sino ang "applicant" - ang nagbabago sino ang family members na inilalagay sa form.
- Sa form ng principal applicant, family members nya ang siya ang pipirma
- Sa form ng spouse, family members nya at sya ang pipirma.

And would it really matter? Really does not - marami naman ang gumawa tulad ng pag-unawa ninyo, others do it na ang applicant is the one na pumirma sa form. Consistency lang ng information sa family members ang tunay na aim nitong form - declaring sino pa ba ang eligible na family members pwedeng ma-sponsor down the line, under eligible programs. (Parents, children....etc.)

ok lang as long na nandun ka pag deliver sa office.. same thing din ang ginawa ko kase most of the day, nasa office ako...

Salamat po sa mga ngrespond. Yun nga po sir, sa personal history po at addresses ang ibig ko po sabihin. Kasi nakaleave lang din po ako.. Say for example po: 2007 06 - 2010 03 working, then 2009 04 vacation lang po for 3 days. Include pa po kaya yun? Iniisip ko po kasi siguru po kung 1 month vacation, pwede po. Pero pag ilabg days? Inilagay ko din naman po sya sa travel history. Anu po sa tingin nyu? Salamat po ulit

Hindi ko na nilagay sa backgraound/declaration ang 3 days travel namen sa "your travels" form lang

Salamat po... How about po ang 1 month vacation? Naputol din po employment ko kasi vacation ng 1 month, ilalagay din po yun then continue ulit ng sa work... Example po: 2012 11- 2013 03 employment, then 2013 04- 2013 04 vacation/travelling, tapos po 2013 05- 2013 12 same employment? Ganun po kaya? Pasensya na po. Thanks a lot!

All set na kami to journey on Winter (30 December) for whiter pasture (hindi greener)
Got ticket PAL (12hrs direct) for Vancouver (early booking para medyo mura), we booked for a Hotel (Days Inn Vancouver Airport) through Agoda (Reasonable price), next day Air Canada for Saskatoon. Done lahat through on-line booking (no cash available kase - nice thing may credit cards) ;D

By the way, just done with PDOS last 1 AUG 14... (Hahapin kase ang PDOS sticker sa Immigration pag alis).
Seminar lasted for almost 3 hours.
Smooth lang ang processing kase we arrived early (around 7am - kase maulan ata baka matraffic).
Parked sa Shakeys (Nagpaalam kami para sure na ok lang-then dun na rin kami lunch).
Pila ng kunti bigay valid ID for security ID and get our number (24) then wait for a while (best thing na nag fill-up kami online form kaya di na kami nagfill up) -http://pre-registration.cfo.gov.ph/
Prepare documents in the following sequence: Photo (upper top right side), PDOS form, photocopy of Visa & photocopy of Passport (stapled na dapat kase strict sila pag di nakastaple). Ready na din original Passport.
8AM start na processing (antay tawag number para pakita mga documents) and payment (400 each including our 2 children pero na namin sinama).
After payment, wait kami na mag 9:30 AM para umakyat sa taas for seminar.
Habang antay, may registration ang COMELEC for absentee voting (pde pa bumoto sa 2016 election for national candidates) habang antay mag start and seminar... super lamig sa seminar room (practice daw- d kami nakadala ng jacket).
Nun natapos na seminar, ang facilitator ba mismo distribute ng passport with CFO (PDOS) sticker
Overall... excellent day naman kahit gutom na kase mga 12:30 na natapos...

Try nyo din attend ng COA sa IOM... the best trainor, Serge Bisson....
Pati sa CIIP... great na may action plan...

Paano mo nalaman na kulang pa ng info yung security screening nyo? Kami kse almost 2 months na after ng medical wala pa din update sa ECAS. wala pa din update na MEDICAL RECEIVED. Wag naman sana kami umabot ng one year. Nag email na ako sa VO na completed na namin yung medical pero wala pa din response.

rcg, everyday kami nag checheck sa ecas but still in process pa rin ang naka lagay. Last june 10 2014 pag na in process.balak ko sana mag email sa cem by next week after august 10.natamong ko na din kasi ang sa clinic ang sabi submitted na raw..ikaw medical received na?
